Every time you visit for a checkup or cleaning, just ask and we’ll include an at-home teeth whitening treatment kit. New and existing patients are eligible to receive at-home teeth whitening treatment.
Once you enroll in the program, you will receive:
To be eligible for the program as a Fairway Dental Care guest, you must meet the following requirements:
*Fairway Dental Care reserves the right to refuse on-going whitening treatment if deemed necessary based on patient health conditions, misuse, abuse, or any other factor deemed necessary to void the offer. Lost or destroyed custom trays will be replaced at cost to patient. Guests who lose eligibility will then need to complete 2 prescribed hygiene care appointments and/or restore all decay to be reinstated in the Free Teeth Whitening for Life program. New patient exams must be completed by Dr Falter and not a specialist. No-shows or going longer than 8 months between cleanings will void the offer.
